New Patients

If you are a new patient scheduled for an appointment, it is extremely important that your New Patient Forms are completed upon arrival. If not, you could be rescheduled for a future appointment. This will result in a delay in your treatment and future progress. We will mail your paperwork before your appointment date. You will also need to bring your driver’s license and a copy of your insurance card(s).
